Consultation Informed Consent Agreement

The purpose of this agreement is to explain what it means to engage Dr. Jamie Marich as a consultant and to obtain your informed consent about the process.

  • By definition, clinical work is the diagnosis and treatment and mental and emotional disorders. On the other hand, consultation is a non-clinical relationship. If you engage and work with Dr. Jamie Marich as a consultant to you, then you are not becoming Dr. Jamie’s patient or client for whom will Dr. Jamie will be diagnosing or treating you for any mental or emotional disorders. 

  • What do you need to know about consultation?
    • Consultation may be short term (i.e. a single session) or long-term (i.e., an extended number of sessions based on the consultee’s expressed needs and goals). 
    • Consultation is a collaborative process where Dr. Jamie Marich (the consultant) offers her experience and insight to you (the consultee) for an agreed upon fee. 
    • Depending on your expressed needs and goals, consultation may be… 
      • professional in nature. You may engage Dr. Jamie to obtain experience and insight from Dr. Jamie about your own clinical work and cases, even though such a relationship does not meet the legal parameters of clinical supervision as described by state licensure boards.
      • educational in nature. You may engage Dr. Jamie to learn a new skill or series of skills or obtain Dr. Jamie’s opinion and experience about self-disclosure in professional settings. This type of consultation might involve an experiential component, but it would not create and does not imply that Dr. Jamie is creating a clinical relationship with you. For example, with your consent, you would allow Dr. Jamie to demonstrate a particular skill on you.
      • fall under the purview of spiritual direction. You may engage Dr. Jamie to obtain experience and insight from Dr. Jamie about matters surrounding spiritual development. This type of consultation might involve an experiential component with your consent, but it would not create and does not imply that Dr. Jamie is creating a clinical relationship with you.

  • Dr. Jamie may feel that your needs exceed the scope of the consultation relationship. If so, then she reserves the right to refer you to a clinical professional in your state or country as a condition for continuing with the consultation relationship. 

  • Dr. Jamie reserves the right to terminate a consultation relationship for any reason. If the consultation relationship has been a long-term one, then Dr. Jamie will give you reasonable notice and a recommendation to another consultant if you wish to continue a consultation relationship with someone other than Dr. Jamie. 
    • If you require a letter of recommendation about your work from Dr. Jamie, then you must give her 2 weeks’ notice. If you require a letter in less time or if you lose a letter and need it replaced, Dr. Jamie reserves the right to charge you for her time and the office’s time in preparing the letter. 
    • If Dr. Jamie Marich does not believe that she will be comfortable to write you a positive recommendation (especially for EMDRIA certification), she will let you know as soon as possible about what she would like to see you address or adjust if she is to write you a positive letter.
